RESIDENTIAL SERVICES 

We know that “home” can mean something different to everyone. That’s why we’re committed to helping your loved one find the home that’s best for them, providing the support and services needed for successful community living.

Our experienced staff works with individuals, families, and neighborhoods so that the dream of living in the community can become a reality.

Group Living

Currently, LOGAN has seven group homes located in various South Bend neighborhoods. Services include:

  • Residential settings consisting of 7-8 individuals living in a home-structured atmosphere

  • 24/7 supervision by trained staff

  • Assistance and training in daily living skills

  • Access to community and recreation activities

 

Supported Living

Many individuals live in apartments or homes of their own with support from LOGAN staff. Residential settings are located throughout St. Joseph County and consist of 3-4 individuals living in a home-structured atmosphere.

Support Living Services include:

  • Supervision by trained staff based on your loved one’s identified needs

  • Assistance toward goals and life skills

  • Access to community and recreation activities

Financial Support

Group living can be funded by Medicaid fund services. Supported Living can be funded through a Community Integration and Habilitation waiver.
