Caring for a family member or loved one with intellectual or developmental disabilities is challenging. We’re here to help you. Our Family Supports services touch on multiple areas of concern in order to enhance the quality of life for your loved one, as well as support everyone involved in their care. Family Supports can be funded wither through a Family Supports or Community Integration Habilitation waiver.
Integrated Living Skills
LOGAN provides services for adults and children whose impairments have delayed or blocked developmental skills, creating a barrier for activities of daily life. Whether these delays are cognitive, social, fine motor, gross motor or mobility, our Community Habilitation program will improve these skills and enhance your child’s quality of life. Group or Individual Community Habilitation Services are available. The duration of support is variable and based on individual needs and desires.
Residential Habilitation Service will help your loved one live comfortably and safely in his or her community. Here we help with personal care, getting to and from doctor visits, housecleaning, grocery shopping, cooking, and recreational activities. This is a service for people on the Community Integration Habilitation (CIH) waiver.
Participant Assistance Care
Similar to Residential Habilitation Service, Participant Assistance Care (PAC) will help your loved one live comfortably and safely in his or her community. Here we help with personal care, getting to and from doctors’ visits, housecleaning, grocery shopping, cooking, and recreational activities. This is a service for people on the Family Supports (FS) waiver.
Caregivers need support. Sometimes that means giving you a chance to step away for a bit. Our trained volunteers and staff will give you a break from your usual routine. Whether we take your loved one out of the home on a special outing, or stay at home to give you some time away, you can trust that our respite staff will care for your loved one while you are away.
